High-Grade Non-Hodgkin’s Lymphomas

The term “high-grade non-Hodgkin’s lymphomas” refers to three histologic subtypes categorized in the Working Formulation1 classification (Table 10–1). At that time, the definitions of low-, intermediate-, and high-grade lymphomas were based on differences in survival, and those with the worst prognoses were considered the high-grade histologic subtypes of small noncleaved cell lymphoma (SNCCL), lymphoblastic lymphoma, and immunoblastic lymphoma, which had survival rates ranging from 23 to 32 percent. Parodoxically, with the advent of dose-intensive multiagent chemotherapy programs, the survival rates for the high-grade lymphomas are now generally superior to those of intermediate-grade lymphomas. Despite improved histologic techniques and molecular markers, certain entities such as the Burkitt’slike subtype of small noncleaved cell lymphomas or immunoblastic lymphoma cannot reproducibly be distinguished from large B-cell lymphoma. Although the Burkitt-like subtype continues to be recognized in the Revised European-American Lymphoma (REAL) classification,2 most hematopathologists disagree as to whether immunoblastic lymphoma constitutes a separate entity from diffuse large B-cell lymphoma.3 With these considerations, this chapter focuses on the REAL classification entities of Burkitt’s lymphoma, Burkitt-like lymphoma, and (in the case of precursor Tor B-cell neoplasms) lymphoblastic lymphoma.
